What is an Incident?
An incident is any event that requires documentation, investigation, or response. Incidents can range from minor issues to major emergencies requiring full incident command.
Common Incident Types

Reporting an Incident
Quick Report
- Tap Report Incident from the dashboard or More menu
- Select the incident type
- Add a title and description
- Optionally add:
- Location - Where it happened
- Photos - Evidence or documentation
- Involved parties - People involved
- Tap Submit
From SOS
If you triggered SOS, you can convert it to an incident report after resolution:
- When SOS is resolved, you’ll be prompted to create an incident
- The incident pre-fills with SOS details and location
- Add additional information as needed
- Submit for review
Incident Status
| Status | Meaning |
|---|---|
| Open | New incident, needs attention |
| In Progress | Being investigated or addressed |
| Pending Review | Awaiting supervisor review |
| Resolved | Issue has been addressed |
| Closed | Fully documented and archived |
Incident Details
Each incident includes:
- Timeline - Chronological activity log
- Location - Map showing where it occurred
- Attachments - Photos, documents, evidence
- Involved Parties - People connected to the incident
- Notes - Internal notes and updates
- Related Items - Linked tasks, forms, or other incidents
Managing Incidents
Viewing Incidents
- Open Incidents from the More menu
- See incidents in a list sorted by date
- Filter by status or type
- Tap any incident to view details
Updating an Incident
- Open the incident
- Tap Add Update to add notes or photos
- Change status as the situation progresses
- All updates are timestamped and logged
Incident Forms
Link custom forms to incidents for structured data collection:
- Open the incident
- Tap Forms tab
- Select a form template to fill out
- Submit the form - it’s automatically linked to the incident
Notifications
Incident notifications are sent to:
- Assigned users - When assigned to an incident
- Supervisors - For high-priority incidents
- Incident Commander - For ICS-activated incidents
- Team channels - If configured
Critical and high-priority incidents trigger immediate push notifications and cannot be muted.
Common Questions
Who can create incidents?
All team members can report incidents. Permissions for editing and closing are controlled by your organization’s role settings.
Can I link an incident to a task?
Yes. From the incident detail view, you can create a follow-up task or link to an existing task.
How long are incidents stored?
Incidents are retained according to your organization’s data retention policy. Contact your admin for details.
Can I reopen a closed incident?
Contact your supervisor to reopen a closed incident.
